Volunteer Waiver

All volunteers are required to complete this waiver before participating in activities at the Evanston History Center. This waiver is intended to inform Volunteers of risks associated with volunteer activities associated with Evanston History Center (EHC), and to release the organization from liability. This waiver is valid for 3 years from the date of the Volunteer’s signature. Please fill out the form below. Completed waivers must be submitted prior to your first volunteer shift.

In the capacity as an Evanston History Center Volunteer, I agree to the following statements:

Volunteer Role *

I am donating my time for this volunteer role and I understand that in this role, I am not an employee of EHC and I will not receive compensation for this donated time.

Volunteer Duties *

I will follow all the rules and regulations of EHC and will follow all directions and instructions given to me by EHC staff and/or certified volunteer leaders.

Conduct *

I will conduct myself in a professional manner and present a positive image of EHC, including being respectful and helpful to guests, other volunteers and staff. I will refrain from using or possessing weapons, alcohol, or illegal substances while engaging in volunteer activities. I recognize that verbal or physical abuse, failure to comply with equal opportunity and anti-discrimination laws, or committing criminal acts is not tolerated and are grounds for dismissal.

Acknowledgement of risks *

I acknowledge that participation in volunteer activities may involve inherent risks, including but not limited to accidents, injuries, and property damage. I will make reasonable efforts to ensure my own safety and the safety of EHC guests, other volunteers and staff.

Liability Waiver *

I hereby release, waive, and discharge the EHC, its officers, directors, employees, agents and volunteers from any and all claims, liabilities, damages, or losses arising from or related to my participation in EHC activities, including any injuries, damages or losses incurred during volunteering.

Photo Release *

I consent to the capture and use of my likeness in photographic images and video or audio recordings taken during EHC activities for promotional use by the organization.
